Here is a great way for kids to set a long term goal for the year and break it into measurable action steps. I have included a progress report for students to evaluate themselves on their progress at each step of the way. Also included is a form for parents to add their input and ways they can support their student. This would be a great tool for Parent-Teacher conferences.
