Note: Only the name section is editable.
To Use: Type your names and print on cardstock or plain paper to make your punch cards. There is a light blue square. Type your student names in that square. The form field goes away when printing.
Pro Tip: Opening the editable PDF directly from a browser will not work well. Most browser based PDF viewers will not know how to display the form fields correctly. Download the PDF to your computer, save it to your desktop, and then open the PDF. If you're having issues, make sure you have the latest version of Adobe Reader.
