Get to know your students at the beginning of the year. This bilingual Student Information Form in English and Spanish will help you connect with the parents and guardians of your students. It's a student information resource you can refer back to all year long.
I use this parent survey at the beginning of the school year and I also hand it out any time during the year that I get a new student in my class. I also used it during meetings with parents and for intervention meetings.
★Print them on colored paper, bright colored paper, the brightest colored paper you have. Neon is nice. That way it stands out from the rest of the papers that parents get from school at the beginning of the year, so they’re more likely to see it, fill it out, and return it quickly.
★When the students return the forms I would give them a little prize. I taught 3rd grade, and prizes were still a big deal at that age. I read the student surveys, respond to the parents, and kept the form in the student’s individual file. It’s handy for you to look at whenever you have parent conferences, or if a parent calls with a concern or question, or just in having conversations with them about their child’s aptitudes.
